The church is located in the village of Girdiškės (Bijotai sen.), in the municipality of Šilalė district, on the road 4109 Upyna-Bijotai-Bardžiai, 8 km north-west of Skaudvilė. It is the only church in Lithuania dedicated to the Blessed Virgin Mary.
The church has a cross-plan, two-domed structure. The churchyard fence is made of wooden barrels, the gate is decorated with an arch made of oak branches.
The church has two impressive side altars. They were created by Kazimieras Andriukaitis, who was a priest in Girdiškės for 30 years. The altars are composed of barked oak trunks and branches. Andriukaitis was also in charge of the construction of the Telšiai Seminary, and was a skilled builder, calculating the number of bricks needed for the building so that only two were left. He also built a machine for the parishioners of Girdiškės for threshing and cleaning grain, a machine for digging peat, and a machine for burning lime.
